Please Note the Following:

1. If any part of this application is not completed, it will NOT be reviewed for award consideration.

2. Application deadline is March 1st.

3. Applicants must submit a current student copy of their transcript.

4. Scholarships awarded may be divided equally for two (2) semesters, provided grade requirements are maintained.

5. Students must be enrolled in and completed a minimum of twelve (12) credit hours.

6. Applicants will be notified by mail or email concerning their scholarship status.

7. This form may make you eligible for multiple SWOSU Foundation scholarships.

8. All Foundation scholarship recipients must sign a scholarship acceptance form and provide a "Thank You" letter to the donor. This letter must be submitted to the Foundation office and will be forwarded to the donor of the scholarship. If the scholarship application applicant does not meet all the requirements, the applicant forfeits the award.

9. The information on this scholarship application may be provided to the donor of the scholarship funds.
